What we are working on

Deliver testing/monitoring/screening/lifestyle on an annual basis to all those with Pre diabetes or at risk of Pre-diabetes.

Emerging Cluster allocation priorities 2016-17
- Improve cluster sustainability through GP recruitment
- Continue to deliver and evolve a pre-diabetic service for the population.
- Support training and development opportunities to increase workforce resilience and skill mix
- Provide protected nurse time to enable a mechanism to exchange good practice and provide peer support across the cluster.
- The cluster network achieved a number of priorities during 2015-16 including:
- Development of a Pre-Diabetic Service that provides, monitoring, screening and lifestyle advice, to those at risk of developing diabetes. Bespoke training for practice staff was delivered to enable proactive lifestyle advice to be offered and the development of a pre- diabetes information leaflet.
- A cluster flu champion identified who encouraged and supported the cluster working through peer review and sharing of good practice which increased uptake of flu vaccine in eligible groups.
- Anticipatory care model rolled out to all nine practices, to proactively manage vulnerable patients who have complex care needs, who are at risk of losing their independence, avoiding unnecessary admission to hospital or Long Term Care Homes.
